About Us
History of ACF
The Alice Carroll Foundation, previously known as Community Living Options, is a new non-profit in Northern California.
Community Living Options was founded by Sara Carroll 48 years ago. CLO has successfully provided 24/7 care for many of our local consumers in the Butte County area. In addition to providing Supported Living Services, CLO helped to develop the practice of writing Personal Care Guidelines or PCGs for disabled individuals of all ages. The team at CLO has worked on updating and expanding our original PCG report and they will now be known as the Facilitated Personal Care Plan or FPCP. FPCPs facilitate safety, prevent abuse and enable continuity of care when a primary caregiver is unable to provide care in the event of an emergency. In 2022, Sara decided to retire part time. She transitioned the Supported Living clients to another vendor, but she wasn’t ready to fully retire.
It has been Sara’s dream for many years after reading Al Etmanski’s book, The Power of Disability, to start a non-profit corporation to recreate Personal Support Networks for the disabled individuals she read about in his book. Sara’s sister, Alice Carroll (who just happened to have Down Syndrome), lived a beautiful life with a loving circle of support. Alice’s support network included family, close friends as well as extended friends in the community. Her life was safe and secure. Sara wants to recreate networks like Alice’s for other disabled individuals in our community. Sara’s nonprofit corporation is aptly named the Alice Carroll Foundation in honor of Sara’s sister.
What We Do
The Alice Carroll Foundation focuses on supporting individuals with developmental disabilities in Northern California by writing Facilitated Personal Care Plans (FPCP), developing Personal Support Networks (PSN), and assisting with Advance Care Planning (ACP). We honor the Lanterman Act every day by helping individuals maximize their opportunities, build relationships, and become contributing members of their community.
Currently serving Butte, Tehama, Glenn, Plumas, and Lassen Counties.
Our Goals
We look forward to partnering with individuals to support their well being by writing detailed, easy to read care plans; building Personal Support Networks to celebrate their unique talents and build their sense of community; and preparing for a sudden event and ensuring their wishes are followed.
Help Individuals Find Their Voice
Facilitated Personal Care Plans (FPCPs) are written in a first person voice. Writing the plans in such a way allows the individual to feel their wants and needs are being addressed when it comes to their personal care. Each detailed plan celebrates the individual’s unique personality, promotes individual needs and ensures care routines are being followed.
Supporting Care and Safety
When a new caregiver comes into an individual’s home, care instructions need to be easy to read, well organized and comprehensive. FPCPs empower individuals with feelings of stability and promote personal safety with clear, concise instructions on how to provide intimate care safely and respectfully.
Creating the Good Life
Personal Support Networks (PSNs) are about creating the “Good Life” for individuals with developmental disabilities. ACF facilitates the formulation of PSNs to ensure everyone knows the loving support of family and friends, contributes and participates in a caring and inclusive community, and accomplishes their lifelong dreams and goals. PSNs provide individuals with opportunities to have a well-planned future and enable their families to have peace of mind.
Preparing for the Unknown
Advance Care Planning (ACP) is thinking and talking about future healthcare decisions; as well as preparing for a sudden event, like a car accident or illness. A person close to them would need to make choices for them; only if they were unable to make their own decisions. This person would use a document called an Advance Directive.
